What to send
- Signed order (script) for pulmonary rehab
- Insurance card, front and back
- Photo ID, such as a driver's license
- Recent records: office notes, PFTs or spirometry, medication list
Who to refer
- Adults with COPD, moderate to severe asthma, interstitial lung disease or pulmonary fibrosis, or long COVID
- Commercial insurance plans, billed directly. We check coverage before anything is scheduled. Self-pay is also available.
- Medicare Part B in New Jersey and California, in small group sessions.
- Not covered by Medicaid, or by other federal or supplemental plans. See the Coverage Breakdown.
- Not a fit if the condition is unstable or needs on-site monitoring
